Please be sure to arrive 15 minutes early for your first visit to fill out any necessary paperwork. In addition, please bring the following items with you:

• Photo identification
• Your insurance card/information
• Your Patient Form
• Any previous dental or relevant medical history

Please download, print and fill out the following form and bring it with you to your first appointment to save time in the office.

We accept most insurance plans, including Delta Dental, Superior Dental, Cigna, Anthem Blue Cross, and MetLife, for your convenience, and we will process your insurance claim. Please bring your insurance card/information with you to your first appointment. We are happy to look into your dental benefits, but it is ultimately your responsibility to understand and be aware of those benefits.

Insurance will generally only cover part of the cost of treatment, so we ask that you pay any copays at the time services are rendered. It is also your responsibility to pay any bills after your insurance company processes your dental claim.

We accept cash, personal check, Visa, MasterCard, and Discover, as well as HSA cards and bank debit cards. We also accept CareCredit dental financing.
